Program Guidelines
• Membership is valid for 12 months from enrollment date.
• Benefits are non-transferable and non-refundable.
• Cannot be combined with insurance, other offers, or third-party plans.
• Treatment discounts apply only while membership is active.
• Unused services do not roll over to the following year.
• Pay in-full at time of additional treatment to receive the 20% discount.

Program exclusions and limitations:
• May not be used with any other special offers or discounts
• The membership fee is due at time of enrollment and is non-refundable and non-transferable. Treatment started prior to enrollment is not eligible for discount. Treatment after enrollment is eligible for a 20% discount when paid in-full the day of treatment
• Orthodontic treatment is discounted at 10% when paid in-full at initiation of treatment. No discount will apply if using CareCredit or Cherry due to merchant fees.
• For orthodontics and extensive treatment plans, member must remain a plan member for the duration of treatment to retain discount plan benefits.
• Should members choose to use outside financing through CareCredit or Cherry the treatment discount will be reduced to 10% due to merchant fees.
• Membership can only be used for services performed in our office. It is the sole responsibility of the member to maximize benefits by arranging the appropriate appointments within the 12-month membership period. If the benefits are not used, the member will not be entitled to a refund. Renewal payment is due at the beginning of the same month of original joining each year. Membership rates may be reviewed and adjusted on an annual basis without notification. We require 30-day notice, prior to the next annual billing date, for any cancellation requests.
• Periodontal Scaling and Root Planing (Deep Cleanings) are separate from this plan and are eligible for 20% off.
• This plan cannot be used for treatment, which, in the sole opinion of the treating dentist, lies outside their scope and skill set. Referrals to specialists are not included in this plan.
